L-4 dermatomal distribution of lesion consists of erupting, tense, flaccid and ruptured vesicles with marked erythema and tenderness.

History:

40 yrs Male presented with lesion on his left leg. Painful blistery lesions started 2 days back from his left buttock, gradually extended below. Before the rash- he had burning sensation and pain over affected areas.

O/E : L-4 dermatomal distribution of lesion consists of erupting, tense, flaccid and ruptured vesicles with marked erythema and tenderness. Not Diabetic, No major illness. No medication taken for this problem

P/D : HERPES ZOSTER RX : Oral Acyclovir 800mg 5 times daily for 7 days Topical Acyclovir cream 5 times daily for 7 days Tab Trytin 25mg daily for 10 days Per-rectal Diclofenac for 5 days ( Oral Valacyclovir could be a good choice, we don’t have it)
