Differential DiagnosisHistory: 57 year-old American touring Europe. Develops small itchy "spot" and obtains topical cream. Initially, cream makes area better, but then affected area experiences explosive erythema and edema with intense itching. Patient applying steroid + neomycin-containing cream. (Fig 2) Allergy to neomycin confirmed later by patch testing and patient warned about future exposure.
