History: This farmer injured his hand on some wire. He later developed a nodule at the site of the injury and subsequently a discharging plaque. Other nodules then arose along the lymphatics up to the elbow. Bactrial cultures were negative. Tissue biopsy showed subcutaneous suppurative granulomas. No organisms were seen on special stains but culture grew Sporothrix schenckii. The origin of the organism was not known but may have been mouldy hay or rotting wood. He was treated with Itraconazole 200mgs daily. This may be needed for three months.
