Differential DiagnosisHistory: 60 year-old male who suddenly developed weakness (could not get up out of chair or climb stairs) and skin rash. Note color of photo-accentuated rash on chest, typical of DM. Also had typical papules over the knuckles, posterior nailfold erythema, and dystropic cuticles - alltypical of DM. Finally, one focus of calcinosis cutis, actively spitting out calcium spicules. Elevated CPK and aldolase. Work-up for internal malignancy underway.
