Differential DiagnosisHistory:
This is a 21 year old woman . She states she has had repeated episodes of this stomatitis / inflammation of the nasal mucosa and scattered itchy red blisters primarily over the hands every month for the last 3 months associated only with the time of menstruation (and resolving entirely in between episodes). There is some slight dryness around the eyes but no obvious rash or discharge.
She is systemically well. No temps or weight loss. No bowel symptoms or bleeding PR. No new medications or other exposures. No regular medications. No previous personal or family dermatological history of significance nor history of inflammatory bowel disease. I took 2 x 4mm punch biopsies today from one of the hand lesions for culture and histopathology plus have sent swab for viral PCR and m/c/s. I haven't sent any samples for immunofluorescence at this stage. With a recurrent rash, like this, you would have to suspect a drug reaction of some sort.
