History:

 A young adult woman presented with slightly painful and indurated crusted rounded ulcer about 1.5 cm in diameter with red border involved the flexor aspect of the forearm of 2 weeks duration. Cloxacillin 250 mg four times daily cured the lesion within 10 days.
