Differential DiagnosisHistory: A middle-aged man presented with more than one year history of gradual and slowly progressed nail discoloration, distal onycholysis and partial nail dystrophy involved most of the fingernails particularly thumbs' nails. Clinically, it was difficult to differentiate between nail psoriasis and onychomycosis, fortunately nail clipping with KOH examination yielded positive fungal products. Two months course of oral Terbinafine (Lamisil) 250 mg per day gave excellent result.
