History: A young adult diabetic man with juvenile onset diabetes mellitus since early childhood. He developed an atrophic plaque with hyperpigmented border on the dorsum of the right hand of one year duration. The surface of the lesion was shiny and waxy with few telangectases. Skin biopsy findings were in keep with necrobiosis lipoidica diabeticorum.
