History: This young man with dental caries and periapical abscess affecting a molar tooth developed an orocutaneous fistula. Orocutaneous fistulas or cutaneous sinus tracts of dental origin are pathologic communications between the cutaneous surface of the face and the oral cavity. Chronic dental periapical infections or dentoalveolar abscesses cause the most common intraoral and extraoral fistulas. Treatment usually includes extraction of the affected tooth, pulpotomy, or pulp removal and drainage. Plastic or oral and maxillofacial surgery can be performed to address scarring caused by the cutaneous fistula.
