History: A 20-year old male suffered closely arranged grouped papules with black central keratinous plugs in a linear fashion on the central upper part of the forehead since early childhood. At presentation, an intervening honeycombed atrophic scars were interspersed between comedo-like lesions with few inflammatory papulopustules in the nearby areas in addition to evidence of associated acne vulgaris lesions on other parts of the face. The lesion in this patient is localized to the upper central area of the forehead (bilateral symmetrical distribution) on contrary to most cases of nevus comedonicus which favor unilateral localization.
