History:

This man presented with chronic onychomycosis. On examination, mild paronychia, loss of cuticle of some nails, dirt-like yellowish-green nail pigmentation, subungual debris and dystrophy of some nails were seen. The condition started one year ago and involved both fingers’ and toes’ nails. The patient had no history of previous drug intake, diabetes mellitus or other causes of immunosuppression. The clinical picture in the presented case was closely mimicking the yellow nail syndrome, however the patient was old and had no associated respiratory tract infection or lymphatic abnormality in addition to nail clipping yielded positive result for fungi.
